Hosted at: MetroHealth Parma Medical Center 12301 Snow Rd, Parma, OH 44130 YOU MUST PRE-REGISTER Tuesday 11/10 - 4:00-6:00 - Code: PARMA1 Tuesday 11/17 - 4:00-6:00 - Code: PARMA2 Tuesday 11/24 - 4:00-6:00 - Code: PARMA3 Register: https://ccbh-poi-testing.azurewebsites.net/ or Call: 216-957-3939 (Tues-Fri 8:30-4:00) Testing will be provided by drive thru or walk up. Testing is provided for individuals age 2 & older. Youth must have guardian on site with them to receive testing.
